I, the undersigned, give my consent to be transported by Bridge City Church during the Mission Trip and will assume all liability for their participation in this activity/event and any injury that may result during the transport or at the event/activity.

I will not hold Bridge City Church or anyone acting on its behalf responsible or liable for injury occurring to the named person(s) in the course of such activities or such travel. This Waiver and Release will be valid for all transportation occurring as of and after the date below.

I certify that my child has permission to participate in the Buffalo Mission trip with Bridge City Church. I understand and agree that if my child is age 12 or younger, they will be accompanied and supervised by a parent/guardian or an adult. I understand it is my responsibility to arrange in advance for the person to be responsible for my child, and that they will need to submit an application and pay the fee for the mission trip.

I understand and agree that this release shall hold harmless Bridge City Church and any leader or responsible person involved in the mission trip from any and all liability relating to my child, from all personal injury or illness that my child may suffer, and from any loss of property that may occur to my child during this mission trip. In case of emergency, I give permission to the mission trip leader(s) and representatives to obtain medical treatment for my child in my absence.

Mission's Refund Policy

All mission trip expenses incurred, including the application fee, are nonrefundable but can be applied toward another trip.

Bridge City Church Mission's Policy

All applications will be reviewed by the Elders of the church and the Missions Coordinator. Applicants will receive an acceptance or denial within an appropriate timeframe.

What Does It Mean to Be Sent?

Being sent by the church body on a short-term mission trip means:

You are commissioned by Bridge City Church to accomplish a specific task in a specific location.
The church affirms (“says Amen” to) your life, character, and gifting.
The Elders agree that:
- You meet all requirements
- The timing is appropriate for your current spiritual season

Required Character Qualities

Individuals being sent must demonstrate:

Spiritual maturity appropriate for the mission
Active service and consistent faithfulness in the local church
A stable relationship with Jesus (evidenced through prayer and worship)
Adults: Family life in order
Youth: Submission to parents
Employment (if age-appropriate) and a strong work ethic
Faithful service within the church
Biblical stewardship practices
Faithfulness in tithes and offerings (including accountable giving for youth)
Good financial management and freedom from excessive debt
A teachable spirit
Willingness to participate in training and preparation

Examples of readiness include:

Participation in evangelism and ministry training
Consistent attendance at:
- Sunday services
- Connection Groups
- Bridge City Students
- Other church functions

Personal Responsibilities

Each applicant is responsible for making practical, advanced preparations, including:

Securing vacation time (if applicable)
Making financial sacrifices
Conducting appropriate research

Mission Procedures

All applicants must adhere to the following:

Submit all forms and applications on time
Accept full financial responsibility for the trip
- Financial assistance may be requested from the Elders
- Requests must be submitted at least 60 days prior to departure using the proper form
Ensure full payment is completed by the departure date
Submit a $50 non-refundable application fee with the application

Financial Assistance Information

Applicants may request financial support from Bridge City Church
Each request is reviewed individually based on need
Requests should not exceed 50% of the total trip cost
Requests must be submitted 60 days prior to departure
Financial assistance is available only to:
- Bridge City Church members
- Their children
A $50 non-refundable application fee is required and will be applied toward the trip cost
Full payment must be completed before departure

Missions Policy and Refund Statement

I have read and understand the Missions Policy and Refund Statement of Bridge City Church. I am prepared spiritually, I have planned accordingly, and sacrificed financially for this trip.
